Fountain Medical Centre are looking for a Salaried GP for 4 / 5 sessions per week to work over Monday/Tuesday and Wednesday.

We are looking for a dynamic and forward thinking GP interested in a salaried position to join our friendly well established practice. We hope to recruit someone who will contribute to providing high quality patient centered care to our patient population. Support for CPD and special interests, generous holiday and study leave entitlement.

The ideal candidate should have excellent people, planning and organisation skills. You will need to show flexibility and be able to work under pressure as well as being able to manage your own workload.

Successful candidates will receive an attractive package of competitive salary, commensurate with experience and qualifications, NHS Pension, allocated study leave in addition to a generous leave entitlement (pro rata) plus statutory bank holidays.

Main duties of the job To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the PMS contract. Furthermore, the post-holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team. The practice has weekly MDT meetings to encourage team working.

About us We are a dynamic, forward thinking practice wishing to deliver locally innovative and integrated services to our practice population of approx. 17500 patients. We are a SystmOne practice based in Morley, Leeds and our last CQC rating was good.

Our clinical team of GP's, ANP, PA's, practice employed Pharmacist and large nursing team work alongside an evolving team of network employed pharmacists, pharmacy technicians, first contact Physiotherapists, care co-ordinators/social prescribers which reduce our workload.

Job responsibilities Clinical practice

Provide a full range of primary care services and assist in the service and delivery of the care management of the practice population

Take a reasonable portion of the clinical workload within practice time including responsibility for booked surgeries, urgent patients and home visits as appropriate

Participate in primary care development projects and ensure that personal professional development reflects the training and development needs of such projects

Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions,

Prescribe and review medication appropriate to patient needs and in accordance with practice guidelines

Provide information and advice to patients on medication regimens, side-effects and interactions and work with them to support adherence.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that promote them to live healthily, and apply principles of self-care

Protect and preserve patient dignity, privacy and confidentiality and comply with NHS requirements in relation to obtaining informed consent

Communication

Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Anticipate barriers to communication and take action to improve communication

Maintain effective communication within the practice environment and with external bodies when required

Actively participate in practice team meetings.

Delivering a Quality Service

Recognise and work within own competence and professional code of conduct.

Be aware of and promote the content of the practice Patient Charter

Produce accurate and complete records of patient consultation, consistent with legislation, policies and procedures

Prioritise, organise and manage own workload in a manner that maintains and promotes quality

Participate in shared learning across the practice and wider organisation

Participate in the management and review of patient complaints, and identify learning from clinical incidents and near-miss events utilising a structured framework

Interpret national strategies and policies into local implementation strategies that are aligned to the values and culture of general practice

Personal and people development

Take responsibility for own development, learning and performance. Agree a personal educational development plan with the Partners which benefits both personal and practice objectives

Support the development of others in order to maximise staff potential

Participate in the development of local guidelines, protocols and standards

Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of Working Smarter

Ensure clear referral mechanisms are in place to meet patient need

Management of risk

Apply infection-control measures within the practice according to local and national guidelines

Participate in the local implementation strategies that are aligned to the values and culture of general practice

Use technology and appropriate software packages as an aid to the planning, implementation and monitoring of care

Use accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes

Understand the responsibility of self and others regarding the Freedom of Information Act

Learning and development

Assess own learning needs and undertake learning as appropriate using the allocated development time

Maintain a high standard of continuing personal development and participate in annual appraisal/revalidation

Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning

Health, Safety and Security

Know the health and safety policies and procedures within the workplace.

Use the personal security systems within the workplace according to practice guidelines

Use appropriate infection control procedures and maintain work area so that it is clean, safe and free from hazards reporting any potential risks identified to the appropriate person

Participate in Health, Safety and Security training when required

Equality and diversity

Act as a role model in the observance of equality and diversity good practice

Support people who need assistance in exercising their rights

Qualifications

MRCGP

Bachelor of Medicine; Bachelor of Surgery

GMC registered with a licence to practice.

Registered on the NHS Performers List.

Competent IT Skills

DFSRH

Minor Surgery

Comfortable with SystmOne clinical System
