Requirements
* Highly motivated, ambitious, and passionate about delivering high‑quality care services
* Strong attention to detail and accuracy when managing schedules and reports
* Ability to work collaboratively with Care Managers, care workers, and other team members
* Previous experience as a Care Coordinator in Health & Social Care (minimum of 2 years)
* Experience with electronic care planning/rostering systems
* Proven ability to manage and maintain a high‑quality, dynamic care service
* Excellent people management and communication skills
* Strong leadership and teamwork abilities
* NVQ Level 3 in Health & Social Care or willingness to work towards it
Key Responsibilities
* Ensure that care worker visits are delivered in line with Hartwig Care’s commitment to continuity and flexibility.
* Ensure all client visits take place on time and are allocated to the most appropriate care workers based on individual needs.
* Provide care workers with all necessary and up‑to‑date information regarding client visits, service delivery expectations, and company policies.
* Receive and record information and reports from care workers—both verbally and in writing—within agreed timelines.
* Coordinate and schedule care worker review meetings and team meetings, ensuring they take place as planned.
* Cross‑check actual visits against planned bookings to ensure accuracy and consistency in the rota.
* Support ongoing improvements in service monitoring, quality assurance, and client safety.
* Assist Care Managers in communicating daily updates, visit changes, and key information to care workers.
* Manage incoming phone calls and text messages, taking messages and redirecting communications as appropriate through the telephone system.
