Band 6 District Nurse (In-Reach Flow) Role Overview We are seeking an experienced and motivated Band 6 District Nurse to join our In-Reach Flow team. This role will be based within acute hospital sites and will focus on facilitating timely and safe patient discharges, particularly for patients from Midlothian. The post holder will ensure smooth transitions between hospital and community care, supporting patients who are either returning to or entering the District Nursing Service. Key Responsibilities Coordinate and facilitate patient discharge from acute settings to community care. Act as a liaison between hospital teams and Midlothian District Nursing Service.
