Your role will be to support people in living well in Merton, maintaining their independence, staying at home, and improving their safety and quality of life.
This support may include connecting people socially within their communities, making physical adjustments such as installing handrails, or facilitating care at home.
You are empowered to help individuals make informed decisions about their care.
The ideal candidate will have knowledge and experience of the physiological changes associated with aging and their impact on mental and physical wellbeing, which will inform their practice. Additionally, there will be opportunities to utilize understanding of various mental illnesses to develop personalized Care and Support Plans centered on individual needs.
