We have opportunities for a highly motivated Occupational Therapist with a patient-centred approach and a passion for rehabilitation to work within the Community Therapy, Bedded Care Therapy and Rapid Response services, with involvement in therapy triage.
Responsibilities
* Work within an integrated, multi-disciplinary team to provide community assessments and goal-focused rehabilitation to support patients in Pathway 2 bedded care facilities, patients\' own homes and care homes.
* Develop specialist therapy assessment and treatment in a community setting, alongside developing generic skills to provide a comprehensive assessment for a variety of complex patients.
* Travel across the High Peak and surrounding area; access to a vehicle is essential.
* Ensure behaviour and values reflect those set out in the DCHS Way.

Qualifications
* Essential: HCPC registration
* Essential: Diploma/Degree in Occupational Therapy
* Essential: Varied Band 5 clinical experience that must include a physical setting
* Essential: Evidence of CPD
* Desirable: Band 6 experience in a relevant clinical field
Experience
* Essential: Case management experience
* Essential: Good understanding of the NHS and relevant national standards
* Essential: Community and bedded care experience working with adults and older people
* Desirable: Experience of working in rapid response with triage interest
Skills
* Essential: Ability to be an autonomous practitioner
* Essential: Commitment to the clinical development of others
* IT skills
* Able to work across daytime shifts and weekends and bank holidays
