Overview
As the Social Worker within the multidisciplinary metabolic team, you will support children and their families through complex care pathways, helping them navigate clinical, emotional, and practical challenges. You will work closely with consultants, dietitians, and allied professionals across hospitals and community settings.
Key Responsibilities
* Provide social work support to children and families with inherited metabolic disorders.
* Collaborate with consultants, dietitians, and allied professionals across hospital and community settings.
* Assist in care pathway coordination and discharge planning.
* Advocate for patient and family needs within the multidisciplinary team.
* Document social work assessments and care plans.
Qualifications and Experience
* Qualified social worker with appropriate registration.
* Experience in paediatric or metabolic care is desirable.
* Demonstrated compassion, empathy, and strong communication skills.
* Ability to work collaboratively in a multidisciplinary team.
