We are looking to recruit a Reablement Co-ordinator on a permanent basis on a 37-hour contract who truly cares and is committed to making a difference in an individual’s life.
Being a Reablement Coordinator means supporting people in their own homes for a short period to assist them in reaching their full potential, which may include managing various aspects of daily living skills, setting clear and achievable goals to help people rebuild their independence and confidence to sustain living at home and in their communities.

You will effectively work within the reablement team to deliver a high-quality reablement service with the aim of promoting and sustaining people’s independence in the community. You will supervise staff and mentor the personal development of Reablement workers.
A driving licence is an essential criterion for this community-based role.
Complete service users' journeys effectively, ensuring coordination and compliance with organisational policies, procedures, and Care Quality Commission regulated guidance. To ensure that the reablement service achieves the best quality outcomes for service users, their carers, and families.
