Job summary
This is an excellent opportunity for a highly motivated and enthusiastic Band 6 District Nurse to join the Adult Community Nursing Team, working flexibly over a seven-day period. The Specialist Practitioner Qualification in District Nursing is desirable, or at least substantial years of experience working at band 5 level within the district nursing service.
The Evening and Overnight District Nursing Team delivers care between 18:00-08:00 across the community of Salford to people in their own homes. We are part of a dynamic, integrated health and social multidisciplinary team delivering high-quality care, advice, support, and monitoring to patients, their families, and staff within community settings. You will work under the direction and supervision of the District Nurse Lead to provide care to patients with complex and non-complex needs, aiming to promote independence and self-management.
You should have excellent communication skills.

Main duties of the job
1. Provide nursing care to patients within the community environment and plan workload for junior staff, supporting the team leader.
2. Participate in delivering nursing care and support to clients within the community setting, working autonomously with confidence and competence.
3. Work under the direction of the Integrated Team Leader, assessing and managing the health needs of an identified population, including end-of-life care and patients with long-term or complex conditions.
4. Utilize excellent communication and interpersonal skills to chair team safety huddles, attend multi-professional and discharge planning meetings, and feedback to ensure seamless patient care.
5. Assess patient care needs, develop, implement, and evaluate care programs for specific patient groups, and support overall patient care.
6. As Sister/Charge Nurse, provide daily clinical and managerial leadership to nursing and support staff, ensuring evidence-based practice.
