Our Elderly Care unit is seeking a dynamic, highly motivated, flexible individual to join and support our nursing team on Ward 17. We are a large established 32 bed ward, which consists of 9 Acute Frailty Assessment beds and 23 elderly care bed. The ward 17 team are devoted to continually improving the quality of care that enhances our patient’s experience. It’s a busy and fast flowing area, but we work together as a team, supporting each other and ensuring everyone is developed to be the best version of themselves.
We are therefore looking for an Assistant Practitioner or Nursing Associate to join our team. You must be experienced in elderly care and frailty and able to lead, support and motivate a team to provide the highest quality patient care and show evidence of effective leadership, organisational and communication skills. Main duties of the job
As part of your role, you will support the Senior Sister/Charge Nurse in ensuring the provision of excellent care with compassion for the patients, relatives and visitors who are cared for within or by the ward, department, service, or team.
You will ensure a high standard of nursing is delivered by self and others based on assessment, planning, implementation, and evaluation of care to meet patients’ needs in accordance with agreed local and National standards and evidence-based guidelines.
You will act as a role model, presenting a positive image of self, team, and the organisation. You will also be responsible for the delivery and co-ordination of care, through appropriate professional practice, delegation and supervision of duties carried out by junior staff

Detailed job description and main responsibilities

5. Set, monitor, and maintain standards of care to improve the patient experience and outcome of care.
6. Proactively maintain patient safety, conforming to health, safety and security legislation, policies, procedures, and guidelines.
7. Communicates all relevant information to the multidisciplinary team internally and with external organisations as required, to ensure seamless care.
