Main duties of the job
The role is boththe delivery of evidence-based practice for patients presenting with a chroniccondition, and the provision of preventative health care to the practicepopulation. for the care delivered to their patients, demonstrating criticalthinking and skills in clinical decision-making in the management of patients.
The role is nota regular 9am-5pm job; the job requires innovation, flexibility and commitmentand the postholder will be required to work resourcefully as part of the teamto ensure tasks are completed.

Your job is towork directly with key stakeholders to help us to harmonise ways of working andimprove working practices to improve patient and staff satisfaction. You willbe the key link to the people whose care you are supporting, operating as a goto person to ensure that their care is seamless, they are active decisionmakers in their own care, and that everyone involved is working together. Carecoordinators provide extra time, capacity and expertise to support patients inpreparing for or in following-up clinical conversations they have with primarycare professionals.

Knowledge
* Level 2 qualification in Maths and English (M) Understanding of health and social care processes (E) Knowledge/familiarity with medical terminology (E) Understanding of social determinants of health and how these can be addressed with patients (E) High levels of health literacy (E) Excellent knowledge of Microsoft Office (E)

* Minimum of 2 years experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field (M) Experience of working in a multi-disciplinary setting where influence and negotiation is required (E) Experience of coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes (E) Experience providing advice/signposting to users (E)
