About the role

An exciting opportunity has arisen to join a highly skilled, professional, dedicated and approachable team responsible for responding to deteriorating patients. Working autonomously, the team assess, formulate, implement and evaluate plans of care for acutely unwell, deteriorating adults, escalating to other health professionals as required.

The team has a close working partnership with Critical Care colleagues including a supportive clinical lead intensivist, providing education and training, facilitating continued professional development. The team pride themselves on delivering high standards of care to all whilst actively identifying ways to progress professionally and as a team, implementing new projects to benefit patients within our community.

This role will involve a rotational working pattern, incorporating day and night shifts with a dedicated preceptorship period.

Main duties

* Responding to referrals from track and trigger mechanisms relating to acutely unwell and deteriorating patients
* Assessing, formulating, implementing and evaluating individualised treatment plans
* Escalating concerns to other health professionals and working confidently as part of an MDT
* Undertaking teaching and education for both nursing and medical professionals, undergraduate and postgraduate
* Ensuring patients discharged from Critical Care receive ongoing rehab by monitoring patients following Critical Care discharge, involving members of the MDT if required and providing psychological support
* Support colleagues throughout the hospital with caring for acutely, unwell and deteriorating patients. Initiating hot and cold debriefs following significant events or incidents
* Undertaking an active role on the cardiac arrest team, including completion of ALS certification
* Handling and responding to Call4Concern, escalating through appropriate routes as required
* Supporting Critical Care colleagues with intubations, line insertion in environments out of critical care
* Facilitating timely admissions to Critical Care, supporting with both intra and inter hospital transfers



Essential person specification requirements:

* Significant post registration experience caring for acutely unwell patients in an enhanced care setting inclusive of ED, L1 surgical and medical environments and within Critical Care.
* Experience in undertaking assessments of patients using a recognised systematic approach
* Extensive clinical knowledge in the care and management of acutely unwell adults
* Demonstrates ability to work within a small team and importance of team dynamics
* Registered Nurse with current registration
* Evidence of professional development in relevant acute / critical care area
