The post-holder will provide general medical services managing acaseload and deal with a wide range of health needs in a primary caresetting, ensuring the highest standards of care for all registered andtemporary patients.

The post-holder will liaise with Community Nursing Teams, Community Mental Health Care Teams,Social Services, Statutory and Voluntary Services, Local GP Colleagues, IntegratedCare Board


Main duties of the job

Manage a clinical caseload.

Takepart in leading on key disease areas (monitoring trends in the Quality andOutcomes Framework treatment, staff training and prescribing).

Assist in developing appropriatehealthcare services for a diverse, patient list (practice basedcommissioning, patient participation and health promotion).

In accordance with the practicetimetable, as agreed, the post-holder will be available to undertake a numberof clinical sessions including queries, visiting patients at home, checkingand signing repeat prescriptions, actioning test results, medical reports andexaminations (e.g. Insurance Companies) on behalf of the Practice.

These duties should be performedin a timely manner using agreed protocols.

Act upon most alerts (e.g. QOF)and reminders during a routine consultation i.e.

Provide all GP services requiredas part of the GMS contract held between Great Sutton Medical CentrePartnership and the commissioning ICB, including all core services such asChild Health Surveillance.

Undertake surgeries inaccordance with the access needs of the practice population and thecontractual obligation of the practice.

Respond to medical problems presented by patients, including history taking,examination, investigation, diagnosis, treatment and referral whereappropriate.

Provide appropriate preventative health care and advice.

Job responsibilities

KEY RESPONSIBILITIES:

1. Manage a clinical caseload.

3. Assist in developing appropriate healthcare services for a diverse, patient list (practice based commissioning, patient participation and health promotion).

4. In accordance with the practice timetable, as agreed, the post-holder will be available to undertake a number of clinical sessions including queries, visiting patients at home, checking and signing repeat prescriptions, actioning test results, medical reports and examinations (e.g. Insurance Companies) on behalf of the Practice.

5. These duties should be performed in a timely manner using agreed protocols.

7. Provide all GP services required as part of the GMS contract held between Great Sutton Medical Centre Partnership and the commissioning CCG, including all core services such as Child Health Surveillance.

8. Undertake surgeries in accordance with the access needs of the practice population and the contractual obligation of the practice.

9. Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment and referral where appropriate.

10. Provide appropriate preventative health care and advice.

11. Provide medical support as required to patients referred by nursing staff.

12. Provide free certification in line with Schedule 9 of NHS GMS Regulations (1992).

13. Adhere to the practice prescribing formulary, local and national guidelines.

14. Make professional, autonomous decisions in relation to presenting problems, whether self- referred or referred from other health care workers within the Surgery.

15. Record clear and contemporaneous I.T. based consultation notes to agreed standards.

16. Collect data for audit purposes.

17. Compile and issue computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).

18. Engaging in appropriate lifelong educational and audit activity in order to maintain clinical competence and performance, including the completion of objectives agreed by yourself and the practice, in an annual Personal Development Plan. Your Personal Development Plan must be in line with the Practice Development Plan.

19. Actively participating in the training and development of nurses, medical students and GP

trainees in the practice.

20. Supporting the practice staff and responding to requests for advice and assistance from the practice reception, secretarial and nursing staff.

21. Attend prescribing, palliative care, clinical and practice meetings.

22. Attend clinical meetings on a regular basis, to chair these and present clinical topics as required for the maintenance of educational standards and high quality care. You will receive feedback from clinical partners via peer review and to act upon recommendations and demonstrate change.

23. Provide additional enhanced services to the practices that meet the needs of the practices patient population and maximise the income streams to the practice.

24. Undertake additional training where necessary to provide these enhanced services.

25. Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety, annual QOF Assessment of the Practice.

26. Meet all tight timescales/deadlines for audits and written returns to ensure that the Practice meets quality standards and receives the designated funding (e.g. Quarterly Enhanced Services returns, annual QOF audit etc.).

27. Provide holistic person centred care to all patients, treating all with dignity and respect.

28. Treat all staff with a courteous manner mindful of their needs of dignity and respect.

General Duties

1. Be available to take over the responsibility of patient care from the Out of Hours service on a working day or where applicable maintain responsibility till the appropriate time when the Out of Hours service takes over care in the evening.

2. Continue to work until all clinical tasks have been completed. To make arrangements with others to cover when planning to leave before the end of the session.

3. Maintain the financial security of the practice by ensuring accurate and efficient responses to income generation such as reports, dispensed prescriptions, private forms etc.

4. At all times to behave in a professional way that encourages quality care and the development of a team spirit. 360 degree feedback is used to assess clinicians and you will be required to participate in this.

Qualifications

* Registered under article 10 of the General and Specialist Medical Practice Order
* 2003
* Qualified General Practitioner (completed certificate of Completion of Training CCT)
* Currently on a ICB performers list and not suspended from that list or from the medical register
* Evidence of further postgraduate educational activities in relevant fields
* MRCGP
* DCH
* DRCOG
