Health & Justice Support Time Recovery Worker
Job Overview: This is an opportunity to work as part of the Liaison and Diversion Service in Cheshire. This post will increase the capacity of the Liaison and Diversion service team to respond to individuals that enter the criminal justice system, with a focus on engagement and support.
Responsibilities
* Work as part of a team with the freedom to work independently, providing advice to service users, carers, and professionals who contact the service.
* Work with service users in conjunction with other professionals to devise and implement effective individualised programmes of support.
* Liaise with other agencies as required depending on the person’s presentation; work closely with other health care professionals and community agencies.
* Provide a community outreach service within Cheshire.
* Actively promote the service to external agencies as a positive example of user-led services.
Main Duties
* Work within limitations and escalate to Practitioners as appropriate.
* Support Practitioners, STR Workers and Lived Experience workers and Admin staff as necessary.
* Engage in outreach and collaborative work to assess and address service users’ needs.
Main Responsibilities Summary
* Work as part of the outreach team; assess service users’ needs; engage with service users and families; offer liaison, joint-working and referrals to relevant services.
* Independently undertake face-to-face and electronic screening to identify multiple vulnerabilities of people who have been in contact with the criminal justice system or immigration centre, working to guidelines.
* Utilise appropriate pathways and refer to other services as needed; escalate to Practitioners when required.
Qualifications & Experience
Essential criteria
* Competent in literacy and numeracy.
* Basic knowledge of physical/mental condition and rehabilitation.
* Computer literacy.
Experience
* Experience working with individuals with a complex range of health and social care needs.
* Experience working in a community-based setting.
* Experience implementing personalised care plans or rehabilitation plans.
* Experience of individual and group work.
* Experience working with parenting interventions and with education provisions.
Skills, Knowledge & Abilities
Essential criteria
* Aware of a range of vulnerabilities and their impact on individuals.
* Basic understanding of the Recovery Model.
* Ability to form rapport with individuals and groups.
* Basic understanding of the Mental Health Act 1983/07.
* Ability to maintain professionalism in challenging situations; good organisation/planning skills.
* Willingness to work flexibly and meet travel requirements of the post.
