Job Description
Responsibilities
* Deliver a mix of telephone, face‑to‑face and home visit appointments.
* Understand the patient population to provide high‑quality care.
* Provide both planned and urgent primary care as required.
* Deliver patient‑focused, evidence‑based practice to a high standard.
* Support the wider MDT to ensure patients with long‑term and/or complex health conditions are cared for at the right time, by the right clinician.

Person Specification – Essential
* Skilled communicator with the ability to engage effectively at all levels.
* Well organised and committed to delivering high‑quality patient care.
* GMC registered and on the GP Specialty Register.
* Inclusion on the Performers List.
* Experience in primary or urgent care, or newly qualified with a willingness to develop.
Disclosure and Barring Service Check required.
