We have an exciting opportunity for a highly motivated registered health care professional to join our Integrated Community Leadership Team.
Main duties of the job
This is a full time post and will have leadership responsibility for the Community Therapy Services in Amber Valley. As well as the day to day management of the team the post holder will have a focus on achieving high quality care and improving services. There will also be opportunities to work with the wider Management Team to develop collaborative working with stakeholders from health and social care. As a leader in our organisation it will be your responsibility to ensure that your behaviour and values reflect those required to lead in the DCHS Way. You will need to engage effectively with your team, peer group and external/internal stakeholders to support you in delivering your key performance measures. This will require an inclusive, transparent, open approach to your role, where you are happy to be accountable to others and promote the concept of a leader who is challenging, whilst expecting to be challenged.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire. We employ around 4,200 substantive staff, caring for patients and customers in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people\u2019s own homes and via virtual consultations.

Person Specification
Education Qualification and Training
* Degree or relevant equivalent experience
* Evidence of continuous professional development
Exprience and Knowledge
* Compassionate leadership style to ensure high levels of performance and wellbeing within teams
* Experience in an operational management post delivering reablement led services
* Evidence of working in services directly supporting assessment needs in line with the Care Act
* Effective involvement in change management
* Evidence of innovation and achievement in management practice
Skills and Attributes
* High level of verbal and written communication skills
* Analytical skills with ability to present information clearly, concisely and in a variety of formats
* Well developed ability to engage and a range of stakeholders to achieve shared aims
* Problem solving and decision-making competencies
* Ability to prioritise and work to tight deadlines under pressure with excellent organisational skills
* Well developed interpersonal skills and ability to effectively work with staff from a range of backgrounds and levels of experience
* Able to manage budgets and deliver on targets.
* Able to support and deliver cost and quality improvement programmes.
