Job summary
We are committed to the NHS long term plan to provide a faster diagnosis for people through the faster diagnosis standard to ensure people are told they have cancer or that cancer is excluded within a maximum of 28 days from referral.
The post holder is responsible for providing and coordinating a supportive service for patients carers and their families within the primary, secondary and tertiary settings for those who have, or fear they may have an upper GI cancer.
Main duties of the job
The Post holder will play a pivotal role in delivering the faster diagnosis standard, they will be assessing the rapid access referrals, requesting relevant imaging, procedures and blood tests, following up on investigations performed, review reports and results to help close the 28 day pathway, communicating results to the patient and GP and updating data bases.
The post holder will deliver autonomous expert holistic evidence based clinical care to patients with complex care needs and provision of education of patient's carers and staff. The post holder will act as an expert resource to healthcare professionals.
Works in close liaison with the multidisciplinary team within the primary secondary and tertiary settings, coordinating patient care as necessary.
