Make a Real Difference Every Day We’re looking for an enthusiastic, HCPC-registered Occupational Therapist to join our friendly multi-disciplinary Reablement Team to embed a therapy led, strengths-based and a holistic approach to short term care delivery. The Role As a Reablement Occupational Therapist, you’ll play a vital role in helping individuals regain independence and confidence in their daily lives following illness, injury or a decline in function. Working in people’s homes and community settings, you will deliver high-quality, client-centred reablement interventions to a varied caseload – from older adults to those recovering from hospital discharge, surgery, or managing long-term conditions. You will carry out holistic functional and environmental assessments, set personalised reablement goals, and provide time-limited, targeted interventions that promote self-care and reduce dependence. This includes recommending and prescribing equipment, advising on adaptations and offering practical training to clients, carers and Community Reablement Assistants. Collaboration is key – you’ll work as part of a multi-disciplinary team alongside Social Workers, Reablement Officers, health professionals and families to ensure clear pathways, SMART goals are met and risks are managed. What You’ll Bring HCPC registration as an Occupational Therapist. Strong clinical reasoning and problem-solving skills. Experience in community or reablement settings (desirable).
