Overview

Job summary: This is a new and exciting collaboration between Dementia UK and 6 Primary Care Networks in Sheffield. Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families involved in the care of a person living with dementia with complex needs. Based within multi-disciplinary/agency community teams and working alongside nurses, therapists, primary care, social care and third sector providers, you will also benefit from being part of a Clinical Network of Admiral Nurses. It will be of especial interest to nurses looking to enhance their leadership and communication skills. We are looking for a skilled registered nurse, either already working as an Admiral Nurse, or from an adult/mental health or learning disability background. You will have experience of providing person centred care and possess experience of delivering bio-psychosocial interventions to improve the experience and quality of life of PLD and their families. You will also be required to establish and maintain close and collaborative working arrangements with the local authority colleagues, voluntary organisations and charitable groups who form part of the Dementia in Primary Care Project. Dementia UK will support you to maintain and strengthen your clinical and leadership expertise as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities.


Details

Location: Dovercourt Surgery (host practice) delivering care across 6 PCNs in Sheffield. Post is fixed term for 2 years. Full-time. Salary: £47,810 to £54,710 a year. Date posted: 12 September 2025. Contract: Fixed term.


Responsibilities

* Support, deliver and coordinate relationship-centred dementia care within Dovercourt Surgery and the 6 PCNs involved in the project, to improve the experience of carers/families affected by dementia with complex needs.
* Provide specialist bio-psychosocial assessment and support for people with dementia and families with complex needs.
* Facilitate education and training for staff in the delivery of evidence-based dementia care.
* Provide clinical leadership with policy development and quality improvements in relation to dementia care.
* Consultancy to other health and social care professionals regarding dementia.
* Provide leadership and management for one Band 6 Admiral Nurse involved in the project.
* Develop and support care plans for people with dementia and families including positive risk management.
* Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia.
* Provide care coordination for a clinical caseload of families with complex needs as identified within triage assessment in clinics or via home visit if required.
* Ensure safeguarding of vulnerable adults and compliance with Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007) and Deprivation of Liberties Safeguards (2009) in patient care.
* Advocate for people with dementia and their carers/families to promote rights and best interests.
* Participate in case conferences, continuing health care processes and best interest meetings as required.
* Promote integrated care by working with health, social and voluntary sector services; support advance care planning and end-of-life decisions.
* Manage complex care transitions to reduce crisis events.
* Ensure care delivery aligns with local/national guidelines and host policies; refer to other services when needed.
* Lead on Admiral Nurse steering groups with Dementia UK and partner organizations.


Education, Leadership, Evaluation, and Professional/Managerial

* Education and Leadership: Facilitate dementia training and education for staff; lead formal training events; provide professional leadership and mentorship; develop work streams in the dementia strategy; collaborate with stakeholders to improve integrated care pathways.
* Evaluation and Audit: Support quality improvements, collect and review data, evaluate the Admiral Nurse Service, and contribute to strategy groups and local/district dementia initiatives; ensure accurate contemporaneous data.
* Professional: Ensure practice adheres to host employer policies and NMC Code; participate in CPD; maintain registration and participate in supervision; develop policies/protocols for dementia care within the project.
* Managerial: Undertake staff appraisal/supervision of the Band 6 Admiral Nurse; manage sickness/absence; participate in recruitment; lead Admiral Nurse steering groups.


Qualifications & Experience

* Experience – Essential: Significant post-registration clinical experience supporting people with dementia and their carers/families; holistic nursing assessments; bio-psychosocial interventions; autonomous practice; multi-agency collaboration; supervision/mentoring; audits; staff training.
* Desirable: Service-specific experience; leading or managing a service; quality improvement; service monitoring, reporting and evaluation.
* Qualifications – Essential: Registered Nurse (RN/RNMH/RNLD); evidence of specialist dementia training/education; ongoing CPD; commitment to Admiral Nurse Academy development; postgraduate qualification. Desirable: Masters-level dementia specialization.
