In this vital role, you will work closely with primary care teams and specialist lipid services to support early detection and effective management of patients at risk of or living with cardiovascular disease (CVD). Using your clinical expertise and leadership, you'll deliver seamless care, provide education and support to healthcare professionals, and contribute to research and quality improvement initiatives.

Responsibilities include:
* Proactively identify and manage patients at risk of or living with cardiovascular disease, focusing on hyperlipidaemia and FH.
* Search primary care databases, contact patients for intervention, provide counselling, treatment, and follow-up, referring to specialist lipid clinics when needed.
* Ensure lipid management aligns with NICE guidelines and support primary care teams through education and training.
* Maintain accurate clinical records, code consultations to meet Quality Outcomes Framework standards, and foster strong relationships between practices and specialist services.
* Collaborate across multidisciplinary teams to promote integrated care and effective communication.
* Participate in audit, research, and quality improvement projects, contributing to service evaluation and development.
* Provide leadership by planning daily tasks, responding to queries, and supporting continuous quality improvement.
* Identify cost-saving opportunities related to medication management while upholding high standards of patient care and professional development.
