About the role
We are looking to recruit a Support Worker at our sites based in Sandwell. We are looking for a highly motivated individual who would like to join our team in order to deliver good quality service to vulnerable adults. You will be responsible for looking after service users and supporting them with their daily requirements. We are looking for someone with experience of working with a socially excluded group and some knowledge of mental health issues to work with vulnerable customers. You must be able to work on a shift rota which includes evenings, sleep-ins, weekends and bank holidays.
Key Responsibilities
Deliver and promote a high standard of support to include demonstrating and advising on good practice.
Promote customers’ independence and choice.
Advising and assisting in developing a range of social, leisure, work, education or other meaningful activities as appropriate with customers. This may involve supporting customers to use I.C.T. to access information and services.
Support customers in preparing and cooking meals, including menu planning and budgeting.
Support customers to take an active role within the community.
Establish and maintain links with other health care professionals and agencies.
Undertake the key-working role as designated by your line manager, this includes regular key-work sessions, positive outcomes, reviewing support plans and preparing and participating in review meetings alongside the customer.
Undertake initial and on-going assessments in consultation with the customer, manager and involved agencies.
Promote and uphold the Trident Reach policy on Equality and Diversity.
Staff must maintain the confidentiality of all customers and any staff information they become aware of.
Adhere to the Trident Group Policies & Procedures and Code of Conduct.

About You
A positive attitude around person centred approaches to working with individuals with learning disabilities and associated flexible & reliable
Supervision experience of leading, advising and motivating staff
Good communication Skills
Experience of working within a busy care environment
The ability to work both autonomously and as part of a team
Caring and sensitive Self-motivated
