Job overview
The Paediatric Grenfell Coordinator role involves supporting the coordination of the Grenfell Paediatric service and providing holistic social prescribing/link worker support to clinicians reviewing this cohort of patients. The role requires a dynamic, proactive, and highly motivated individual to manage service processes, performance, and work closely with families from referral to post-review.
Main duties of the job
1. Effective Coordination: Manage the planning, prioritisation, and organization of all activities related to the Grenfell paediatric service to achieve a productive, high-quality service.
2. Administration and Systems Management: Establish, maintain, and monitor administration systems related to service delivery.
3. Point of Contact: Serve as the named point of contact for families, ensuring effective communication and support.
4. Annual Reviews: Ensure all patients are offered a holistic annual review with their named paediatrician.
5. Respiratory Reviews: Ensure all eligible patients are offered a suitable respiratory review.
6. Feedback and Development: Continuously provide feedback to management to support the ongoing development of the service.
7. Relationship Building: Develop strong relationships with voluntary services, link worker teams, GPs, dedicated service workers, and the Grenfell Health and Wellbeing team.
8. Support Access to Services: Support clinicians in helping families access local authority services and maintain a thorough understanding of the available services for this patient cohort.
