Details The role As a Support Worker, working within our Liverpool Outreach service, you will help to promote the health and wellbeing of those you support. This will include providing practical and emotional support; supporting people to connect with their communities, developing skills and confidence and promoting independence. We are looking for individuals with values which align to ours, who are highly motivated and committed to enriching the lives of people we support. As an outreach support worker you will be lone working daily, visiting individuals within their own homes or in the local community to provide support visits, in line with their agreed support plan and funded hours. However you will have daily support from the senior support worker, who will link in daily offering advice and support. As well as contact with your colleagues when needed. There will be opportunity to meet with the wider outreach team for team meetings on a quarterly basis.
Responsibilities
* Carry out outreach support visits alone, visiting individuals in their own homes or in the local community in line with agreed support plans and funded hours.
* Receive daily guidance and support from the senior support worker; maintain contact with colleagues as needed.
* Participate in team meetings with the wider outreach team on a quarterly basis.
Contract and Hours
* Salary: £10,184 per annum
* Hours: 16 hours per week (part-time)
* Shift pattern: Monday – Friday 9am-5pm (including some additional weekends on a rota basis)
