Job Overview


Our service is expanding, and we are offering an exciting opportunity to join our busy Integrated Care Coordination for Urgent Care Team as a Band 7 HCPC / NMC Registered Clinical Team Lead based in Hanley, Stoke-on-Trent. The service is developing a multidisciplinary approach and would welcome applications from AHPs to support this.


We are looking for a passionate and dedicated Clinical Team Leader to provide management, support, clinical escalation, and professional expertise to the team of Clinical Triage staff. The team provides telephone support to health and social care registered professionals requiring assistance with Admission Avoidance. This ranges from signposting the caller to the appropriate service, making a direct referral to another service, or offering immediate clinical advice or treatment across Stoke-on-Trent and Staffordshire. We aim to provide the right care, in the right place, at the right time. As duty manager, you will promote seamless patient care, acting as an ambassador for good quality care and sharing and celebrating team achievements.


You will need to be flexible, approachable, and have a sound clinical background and knowledge. You should be friendly but assertive, confident in your decision-making, and able to challenge when necessary.


This role requires covering a 7-day period, 365 days a year, including weekends and out-of-hours work.


Main duties of the job


1. Work as a key member of the Integrated Care Coordination Team to ensure professionals can make referrals through one access point.
2. Lead a team of Clinical Triage staff, supporting them to work independently and exercise their assessment and evaluation skills to avoid inappropriate hospital admissions.
3. Support the team to adjust care programmes as necessary and initiate evidence-based changes in the patient’s best interests.
4. Act as the first line of clinical escalation for the team.
5. Ensure high-quality, clinical triage using knowledge, skills, critical thinking, and professional judgment.
6. Ensure policies and processes are followed to keep staff and patients safe.
7. Provide immediate advice and operational guidance to team members, analyzing available options.
8. Embed quality within all service delivery aspects, owned by staff.
9. Support staff in developing quality improvement projects.
10. Create a team culture where staff are valued, supported, and able to deliver compassionate care.
