Salary: HC5 - £25,183 to £27,269 pa

The role
Working within the Living Well commissioning team, you will be part of an Accommodation Support Team with a specialist Housing Officer. Your responsibilities include working closely with Adult Social Care and Commissioners to find tailored accommodation and support solutions, enabling people with care needs to find a home and manage their move into it. Liaison with housing and care providers is essential to ensure smooth transitions into new accommodation.
About You
* Good interpersonal and communication skills
* Knowledge of safeguarding principles, and a commitment to equality, inclusion, and diversity
* Knowledge of housing
* Experience working with vulnerable groups
* Ability to manage a caseload with emotional resilience and confidence, working independently and as part of a team
