GPs are often the initial point of contact for patients' health concerns.

They provide care considering a patient's physical, emotional, and social well-being.

GPs treat a range of common and chronic illnesses, managing long-term conditions to keep patients healthy.

They refer patients to hospitals or specialists for more complex conditions or urgent treatments.

GPs work within communities, building relationships with patients and understanding their local context.

GPs are involved in promoting good health, preventing illness, and initiating treatments.

They work as part of a wider healthcare team to discuss and coordinate patient care.


Main duties of the job

Core responsibilities

Delivery of highly effective medical care to the population

Provision of services commensurate with the GMS contract

Clinically examine & assess patient needs from aphysiological & psychological perspective. Plan clinical care, referring patients to services/agencies in a timely manner, utilisingPractice & local guidelines

Prescribing, adhering to local & national guidance

Prioritise health problems. Intervene appropriately includinginitiation of effective emergency care

Manage long-term conditions

Support patients to adopt strategiesthat promote healthy lifestyles

Review medications, following appropriate policies

Maintain accurate clinical records along with good practice,policy & guidance

Adhere to best practice recommended through clinicalguidelines and the audit process

Commit to the implementation of continuousimprovement and quality initiatives

Accept delegated responsibility for specific areas of QOF

Attend and contribute effectively to practice meetings

Apply current legislation, adhere to practice policies & procedures onSafeguarding Adults and Safeguarding Children

Comply with the appraisal process

Prepare and complete the revalidation process

Be an integral part of the general practice team as wellas supporting the wider network and multi-disciplinary team


About us

The Practice currently cares for8,500+ patients cared for by a team of 3 Partners, 2 salaried GPs, an ANP, 2Practice Nurses, a Practice Pharmacist, HCAs and a mix of 10 reception/adminstaff.

We are part of East Elmbridge PCN,along with 6 neighbouring practices.

We are a training practice anddelight in welcoming new students. We also hold monthly staff meetings, open toall staff.

Located in East Molesey, Surrey, withparking provided.

Situated near to Hampton Court Palaceand the river Thames, with a wealth of cultural and social venues.

We are looking for a candidate that shares our ethos and able to start as soon as possible.


Job responsibilities

The following are the core responsibilities of the SalariedGP in delivering health services.

The delivery of highly effective medical care to theentitled population

The provision of services commensurate with the primary carecontract

To clinically examine and assess patient needs from aphysiological and psychological perspective and plan clinical care accordinglyreferring patients to other services/agencies in a timely manner, utilisingPractice and any other local guidelines

Generic prescribing adhering to local and national guidance

Prioritise health problems and intervene appropriately toassist the patient in complex, urgent or emergency situations, includinginitiation of effective emergency care

Effective management of long-term conditions

To support patients to adopt health promotion strategiesthat promote healthy lifestyles and apply principles of self-care

Processing of administration in a timely manner, includingreferrals, repeat prescription requests and other associated administrativetasks

Review medications following the appropriate policies,NICE/local clinical guidelines and local care pathways

Maintain accurate clinical records in conjunction with good practice,policy and guidance

Work collaboratively, accepting an equal share of thepractice workload

Adhere to best practice recommended through clinicalguidelines and the audit process

Support the delivery of anticipatory care plans

Contribute to the successful implementation of continuousimprovement and quality initiatives within the practice

Accept delegated responsibility for a specific area (orareas) or the QOF

Attend and contribute effectively to practice meetings asrequired

Contribute effective to the development and maintenance ofthe practice including clinical governance and training

Be aware of your duties and responsibilities regardingcurrent legislation and adhere to practice policies and procedures onSafeguarding Adults and Safeguarding Children

Ensure compliance with the appraisal process

Prepare and complete the revalidation process to remain fitto practice

Commit to self-learning and instil an ethos of continuingprofessional development across the practice team

Review and always adhere to practice protocols and policies

Encourage collaborative working, liaising with all staffregularly, always promoting a culture of continuous improvement

To be an integral part of the general practice team as wellas supporting the wider network and multi-disciplinary team

There may be, on occasion, a requirement to carry out othertasks, this will be dependent upon factors such as workload and staffing levels


Person Specification


Experience

* Ability to both work autonomously and as part of a multi-disciplinary team
* Experience of working in a primary care environment
* Experience of providing home visits
* Minimum of 12 months as a salaried GP
* General understanding of the primary care contract
* Proven ability to evaluate the safety and effectiveness of own clinical practice
* An appreciation of the NHS landscape and relationship between individual organisations, PCNs and the commissioners
* Full GMC registration
* Performers List registration
* Evidence of current validation
* Evidence of last appraisal
* Eligibility to practice in the UK independently

Qualifications

* Qualified GP
* MRCGP
* Certificate of Completion of Training (CCT)
* Vocational Training Certificate or equivalent JCPTGP
* Clinical trainer or supervisor qualification


Knowledge & Skills

* Outstanding level of clinical knowledge and skills commensurate with that of an experienced GP
* Understanding of the importance of evidence-based practice
* Good MS Office and clinical system IT knowledge
* Understanding of the spectrum of clinical governance processes
* Excellent communication skills (written and oral) including recording of accurate clinical notes
* Knowledge of health promotion strategies
* Ability to work within own scope of practice and understand when to refer to GP colleagues
* Experience of triage and the ability to effectively assess and diagnose
* Ability to listen whilst being honest and caring and be both sensitive and empathetic in distressing situations
* Understanding of the local pathology tests processes and the ability to processing the results with advising patients accordingly
* Ability to assess and manage acute deteriorations /exacerbations of long-term conditions
* Understanding of safeguarding adults and children
* Understanding of the importance of evidence-based practice
* Experience of medicines management
* Knowledge of public health issues in the local area
* Understanding and knowledge of QOF and enhanced service
