Main duties of the job
Core responsibilities
Delivery of highly effective medical care to the population
Provision of services commensurate with the GMS contract
Clinically examine & assess patient needs from aphysiological & psychological perspective. Plan clinical care, referring patients to services/agencies in a timely manner, utilisingPractice & local guidelines

Prioritise health problems. Intervene appropriately includinginitiation of effective emergency care
Manage long-term conditions

The Practice currently cares for8,500+ patients cared for by a team of 3 Partners, 2 salaried GPs, an ANP, 2Practice Nurses, a Practice Pharmacist, HCAs and a mix of 10 reception/adminstaff.
We are part of East Elmbridge PCN,along with 6 neighbouring practices.
We are a training practice anddelight in welcoming new students. We also hold monthly staff meetings, open toall staff.
Located in East Molesey, Surrey, withparking provided.

Processing of administration in a timely manner, includingreferrals, repeat prescription requests and other associated administrativetasks
Review medications following the appropriate policies,NICE/local clinical guidelines and local care pathways
Maintain accurate clinical records in conjunction with good practice,policy and guidance
Work collaboratively, accepting an equal share of thepractice workload

Support the delivery of anticipatory care plans
Contribute to the successful implementation of continuousimprovement and quality initiatives within the practice

Person Specification
Experience
* Ability to both work autonomously and as part of a multi-disciplinary team
* Experience of working in a primary care environment
* Experience of providing home visits
* Minimum of 12 months as a salaried GP
* General understanding of the primary care contract
* Proven ability to evaluate the safety and effectiveness of own clinical practice
* An appreciation of the NHS landscape and relationship between individual organisations, PCNs and the commissioners
* Full GMC registration
* Performers List registration
* Evidence of current validation
* Evidence of last appraisal
* Eligibility to practice in the UK independently

Qualifications
* Qualified GP
* MRCGP
* Certificate of Completion of Training (CCT)

* Outstanding level of clinical knowledge and skills commensurate with that of an experienced GP
* Understanding of the importance of evidence-based practice
* Good MS Office and clinical system IT knowledge
* Understanding of the spectrum of clinical governance processes
* Excellent communication skills (written and oral) including recording of accurate clinical notes
* Knowledge of health promotion strategies
