Young People Leaving Care Outreach Support Worker.
Outreach – hours are between 9am and 5pm, Monday to Friday.
This is a temporary to permanent position. If you are subscribed to the update service and have a short notice period, a quick start is possible through a temp-to-perm placement. You will start quickly and work 12 weeks temporarily, transitioning to a permanent salary afterward.
Salary when permanent is 27-29K depending on experience and level 3.
Positions covering Oldham – Rochdale.

Experience supporting young people and their families is required.
Full UK Driving License is essential with Business Insurance.
Responsibilities include:
* Completing property inspections.
* Supporting young people to develop independence skills.
* Managing their diary and working closely with permanent staff.
Our client is a leading provider of care to children and young people in the area. A quick start is possible if you have a current online DBS.
It is essential that you have experience working within a similar setting and preferably have completed NVQ3 Children & Young People Workforce or a similar qualification within this sector.
Qualifications/Requirements
* A high standard of general education.
* Preferably NVQ Level 3 Health & Social Care – Children and Young People (CCYP) or equivalent.
* Willingness to undertake further training.
Job Purpose
To be responsible for providing support to vulnerable people (service users) for the company.
Main Duties & Responsibilities
1. Assist the young person’s allocated social worker in service delivery, including:
* Providing transport/escort as directed.
* Conducting welcome and induction for young people.
* Ongoing assessments of service users’ needs.
* Providing ongoing support and monitoring.
* Assisting with welfare and NHS issues.
* Preparing reports.
* Arranging appointments with partner agencies.
* Maintaining contact with service users.
* Informing service users about support groups.
* Assisting with move-on processes.
2. Maintain accurate electronic records of service users.
3. Prepare for and attend meetings as required.
4. Provide mediation as needed.
5. Use IT packages effectively to support work.
6. Work cooperatively across multidisciplinary teams.
7. Maintain confidentiality regarding service users, clients, and the company.
8. Contribute to policy and procedure development.
9. Report housing issues and assist in resolving defects.
10. Undertake on-call duties as directed.
11. Participate in office reception duties if required.
