Overview
Our Care Co-ordinator is part of the care team and would be expected to help with care calls where necessary.
Responsibilities
* Allocation of care workers to customer visits using effective rostering, ensuring continuity of care and considering locations, skills required.
* Consulting with patients and determining their needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible patient care.
* Working with the care team to evaluate interventions and identify where and when further ones will be required
* Reading, attending workshops and liaising with professional bodies to stay abreast of developments in the field
* Develop and coordinate healthcare programs
* Communicate between staff, patients and family
* Recruit and train staff and create schedules
* Develop patient goals and monitor progress
* Maintain patient confidentiality and quality care
* Developing care plans and rearranging them as necessary when difficulties arise
* Visiting patients, checking on the care they\'ve received and documenting it accordingly
