Palliative Care Staff Nurse (Band 5) - Access/Community Team/Virtual Ward

Starting Salary: £37,335 WTE (rising to Step 2 £41,424 WTE subject to 12‑month satisfactory performance review) pro rata per annum + Pension + Benefits
Hours: 22.5 Hours per week (weekdays)
An exciting opportunity has arisen for a caring and compassionate registered nurse to join our Hospice Team. You’ll be a Registered Nurse with palliative care experience, excellent interpersonal and communication skills and a proven ability to work as part of a multi‑professional team.
Responsibilities
* Provide specialist advice, guidance and support to patients at home, in care homes and inpatient care.
* Get to know each patient and their family well enough to discuss their feelings, worries and anxieties.
* Advise on symptom control, medicines and treatments, ensuring patients receive the practical help they need from community partners.
* Conduct initial and ongoing holistic assessment of new patients and plan care to address practical, psychosocial and spiritual needs of patients and carers, responding to queries or requests for advice and support from patients, families and colleagues.
* Work in a responsive/reactive role suited to experienced staff nurses with palliative care experience.
* Rotate between the Access team and community setting, initially office‑based; a full driving licence and use of your own car is required for patient home visits.
* Ensure all patients receive support quickly and comprehensively and are triaged to the most appropriate team for follow‑up.
Qualifications & Requirements
* Registered Nurse with palliative care experience.
* Excellent interpersonal and communication skills.
* Ability to work effectively as part of a multi‑professional team.
* Full driving licence and own vehicle for home visits.
* Willingness and ability to undergo a criminal records check at a PVG scheme record‑update level; the Hospice will pay for the joint application.
