Overview

HMP Hull are looking for a qualified Associate Nurse Practitioner and therefore you must be registered with the NMC.


Responsibilities

You will be responsible for the assessment of patient care needs and development, implementation and evaluation of programmes of care, both for a specific group of patients as the named care provider and in supporting the care of all patients. As a Nurse you will assist in the management and organisation of work as required, ensuring effective communication is maintained at all times, providing support for junior staff.

We aim to be flexible in matching your availability with our work patterns and the needs of the service and therefore we are also able to offer part time and flexible working options.

You will play a key role in the education, development, mentorship and supervision and assessment of all learners and unregistered staff. You will undertake nursing interventions consistent with evidence based practice, transferring and applying knowledge and skills to meet patients\' needs.

Person Specification


Education and Training


Essential

* NVQ Level 4 in Health and Social Care.
* Registered with the NMC and have the Associate Nurse qualification.


Desirable

* Long term condition management


Knowledge and Experience


Essential

* Demonstration of experience of working with people with complex health needs, in a health or social care setting.
* In depth knowledge of health of primary care, mental health and addiction support and an ability to work across these areas.
* Experience of work in a multi-disciplinary team setting.
* Knowledge of nursing procedures and practices gained through NVQ 4 or equivalent and Associate Nurse qualification.
* Understanding and application of confidentiality


Desirable

* Previous experience of using or accessing mental health services or caring for someone who has accessed services.
* Knowledge or experience of working within a prison environment
* Previous experience of working with people accessing Substance Misuse services or able to demonstrate an understanding of clinical pathways for substance misuse.
* Experience of supervising others.


Skills and Abilities


Essential

* Good patient assessment skills - knowledge of signs and symptoms of a range of mental and physical disorders and its effects on individuals
* The ability to complete tasks and ability to escalate concerns to senior clinical staff as appropriate
* Well-developed verbal and written communication skills.
* The ability to work as part of a team
* The ability to use initiative and take responsibility for agreed appropriate tasks.
* The ability to work autonomously alongside and an awareness of knowing when to seek support.
* Keyboard skills - computer literate with a working knowledge of Microsoft Office
* The ability and skill to form positive therapeutic relationships
* Support service users\' own decision-making that builds on their strengths.
* Able to maintain contemporaneous clinical records following clinical activities.
