What You’ll Do
As a Registered Nurse leading our Reablement Team, you will take full responsibility for clinical oversight and leadership within the service. Your key responsibilities will include:
1. Leading on initial risk assessments and care planning for all new service users.
2. Completing 72-hour evaluations and weekly reviews of reablement goals, ensuring progress toward independence.
3. Supervising care professionals to maintain high standards of care and compliance with clinical protocols.
4. Providing clinical guidance and support to care professionals across the service.
5. Delivering bespoke clinical training to care professionals, ensuring competency in specialist tasks tailored to individual service users.
6. Acting as the primary liaison with healthcare professionals, clients, and families to maintain holistic care standards.

Qualifications
What You’ll Need
7. Registered Nurse Diploma/Degree with an active NMC Pin.
8. Minimum 3 years of nursing experience.
9. Strong leadership, communication, and clinical skills.
10. Experience in community care, rehabilitation, or reablement services.
11. Commitment to promoting independence and person-centred care.
