This is a full-time role with an immediate start available on an on-going basis. Key Responsibilities: * Conduct comprehensive assessments of individuals presenting with drug and/or alcohol misuse issues. * Provide evidence-based interventions and treatment plans, tailored to the needs of service users. * Manage a caseload of patients with medium complexity, ensuring continuity of care and effective risk management. * Support both community and in-patient detoxification processes in collaboration with medical and psychosocial teams. * Work autonomously and as part of a multidisciplinary team to deliver holistic care. * Maintain accurate, up-to-date clinical records in line with professional and legal standards. * Contribute to clinical reviews, care planning, safeguarding processes, and patient outcome evaluations. * Actively participate in team meetings, supervision, and training activities.
