This is a unique opportunity to be part of the development of a national Critical Care Transfer Service for the people of Wales. Established to provide high-quality intensive care during the inter-hospital transfer of the critically ill patient. This post will commence in February 2026 .

ACCTS Cymru, in partnership with the Welsh Critical Care Network and EMRTS Cymru, is seeking experienced, motivated, and enthusiastic senior retrieval and transfer medicine specialists to support the development of this bespoke service for Wales.

We are now recruiting Senior Clinical Fellow Doctors to join our teams across our two Welsh bases, which are located in Cardiff and Bangor. The service operates 7 days a week

Main duties of the job

ACCTS is a predominately road-based service deployed with dedicated critical care transfer vehicles and equipment. The Retrieval and Transfer physician will deploy in a team alongside a support worker and a retrieval and transfer practitioner drawn from a pool of nurses and allied health professionals with extensive intensive care experience and specific training in inter-facility critical care transfer. ACCTS in partnership with WAA/EMRTS will operate a rotary wing model of retrieval and transfer to support long-distance transfers.

Upon completion of appropriate training and a period of targeted clinical experience, it would be expected that you would then take up a full part in the duty rota. The ability to work both day and night shifts is an essential part of the role.

There may be a need to travel to our various bases, across Wales for education and service delivery requirements and all candidates would be expected to contribute to twilight or overnight cover.

Swansea Bay University Health Board has responsibility for the health of around 390,000 people in the Neath Port Talbot and Swansea areas, with a budget of around £1 billion and employing 12,500 people. We are a University Health Board working in partnership with Swansea University, Swansea School of Medicine, the School of Health Science and the Institute of Life Science. The HB has three major hospitals providing a range of services: Morriston and Singleton hospitals in Swansea and Neath Port Talbot hospital in Baglan, Port Talbot.

The Health Board is part of A Regional Collaboration for Health (ARCH), which is a partnership with Hywel Dda UHB and Swansea University. Aimed at improving the wellbeing and wealth of South West Wales.

Swansea has a range of shops, cozy cafes, great restaurants, art galleries as well as being able to offer good outdoor lifestyle with its beautiful coastline and beaches on the Gower Peninsula. Swansea also has excellent sporting facilities including the Wales National pool and liberty stadium home to the Ospreys and Swansea City football club.

To qualify for this exciting role you must be a doctor with ST4+ equivalent training in Intensive Care Medicine / Anaesthesia who is fully registered with the GMC and who has significant demonstrable experience in the transfer care setting, aligned to the personal specification. It is essential that you are ambitious, highly motivated and able to remain calm under pressure.

Selection for this rewarding role is demanding and consists of several clinical scenario assessments as well as group work and a formal interview. An assessment event will be held at one of the ACCTS operational bases at a date to be confirmed.

Qualifications and Training

Essential criteria

1. Full GMC registration with license to practice and up to date appraisal / revalidation.
2. ST4 or equivalent experience
3. Holds FRCA or FFICM
4. Successful completion of relevant life support / resuscitation / trauma courses previously

Desirable criteria

5. Completed intermediate level training / Stage 2 training in Anaesthesia/ICM OR ST5 equivalent experience
6. Substantive Consultant post within Wales
7. Diploma in Retrieval and Transfer Medicine
8. Fellowship in Immediate Medical Care
9. Higher Degree (MD/MSc)
10. Post graduate qualification in medical education

Knowledge & Skills

Essential criteria

11. Competence in management of acute illness and injury in adults
12. Competent in the assessment, resuscitation and stabilisation of critically unwell patients including emergency anaesthesia, ventilation and invasive monitoring
13. Sound judgement and decision making
14. Ability to manage complex work and prioritise effectively
15. Good understanding of the principles of clinical governance

Desirable criteria

16. Previous attendance on an EMRTS/ACCTS clinical attendant scheme, and/or prior engagement with the EMRTS service. (Such as attendance at EMRTS governance days, training events etc.)

Experience

Essential criteria

17. Demonstrable interest in and understanding of retrieval and transfer medicine
18. Extensive experience of inter- and intra-hospital critical care transfers
19. Experience in post-graduate / multidisciplinary education

Desirable criteria

20. Experience of critical care retrieval and transfer medicine in dedicated service
21. Prior experience with evidence of working within an established pre-hospital aeromedical or road-based service
22. Prior experience with evidence of working within an established secondary retrieval service
23. Prior experience working in within a trauma centre (experience of major trauma, major haemorrhage, massive transfusion)
24. Experience in FAST scanning and / or bedside echocardiography
25. Evidence of relevant teaching/instructing in critical care or transfer
26. Experience using simulation for education
27. Experience as a clinical tutor or lecturer at post graduate level

Personal Attributes

Essential criteria

28. Ability to lead a multi-disciplinary team in both the hospital and transport environments
29. Ability to maintain CPD to a high standard
30. Able to travel to more than one ACCTS base for training, supervision and currency shifts if required

Desirable criteria

31. Ability to speak Welsh
