Offer description

To support Patients withpalliative or end of life prognosis and their families/carers to achieve theirpreferred place of care.

To work under the supervision ofClinical nurse Specialists to develop knowledge and expertise in Palliativecare whilst providing a timely and service.

Assisting Clinical NurseSpecialists to manage a caseload of patients supporting with telephone calls,home visits, bereavement support and symptom management.

To provide clinical leadership tojunior members of the team with support, teaching, and clinical guidance.


Main duties of the job

The RGN will be expected, undersupervision, to deliver high standard of personalised care and identify needsof the patients through holistic assessment.

To use their experience andknowledge of palliative care to deliver appropriate symptom control, clinicaland personal care, and medication administration.

To work collaboratively with theCommunity Specialist Palliative Care Team (CSPCT), GPs and district nursingteams.

To be skilled in communication

To be responsible, support andprovide clinical leadership to junior members of staff and Student Nurses.

To be aware of cost effective andefficient use of resources.

To provide supportive care topatients, their families, and carers.

To monitor patients on an ongoingbasis, to evaluate outcomes and adjust care plans to ensure optimumco-ordination and delivery of care.

Assist the community clinicalnurse specialists with visiting and telephone contact to patients/families asappropriate.

Advise on and facilitatebereavement support for bereaved relatives.

Undertake bereavement visits asappropriate.

To develop and maintain effectiverelationships with all members of the multi-disciplinary team to ensurecollaborative working for the benefit of patient care.

To support the development andimplementation of care pathways and patient information.

To maintain accurate and up todate patient records (including electronic records).

To provide a high standard ofpersonalised and on-going care for bereaved families.

We are a patient centred charitythat delivers palliative and end of life care to thousands of people each yearwith advanced life-limiting illnesses who live in Northwest Surrey. The Hospice also provides support to theircarers and families in patient homes, in care or nursing homes, or through itsIn-Patient Unit in the Hospice, as well as though the Bradbury WellbeingCentre. This role is an essentialcomponent in the Hospice at Home Team which forms part of the CommunityServices Team, supporting and caring for patients in the community and ensuringthat the highest standards of care are delivered.

Person Specification


Qualifications

* Registered Nurse
* Experience of ongoing professional development
* Mentorship qualification or working towards
* Oncology or palliative experience


Personal Qualities

* Excellent interpersonal and communication skills
* Ability to manage workload with competing deadlines
* Ability to work collaboratively as part of a team
* Self- motivated and confident approach
* Motivated, flexible, and adaptable individual
* Car driver/full driving licence
* Have access to a vehicle for community visits and ensure appropriate insurance in place
* Ability to work flexibly over a 7 day period


Experience

* Experience of working in a multidisciplinary team
* Knowledge of palliative care or relevant experience
* Knowledge of national, regional, and local policy in relation to quality standards, targets, and improvements initiatives
* Experience with Multi agency working
* Experience in helping to develop clinical services


Planning and Organisation

* Able to manage a varied caseload
* Able to work autonomously and within a MDT
* Able to plan day to day visits and weekly reviews
* Able to monitor caseload and manage a waiting list
* To be able to anticipate disease progression and plan as appropriate


Knowledge and Skills

* Excellent interpersonal and communication skills
* Ability to motivate and inspire others
* Ability to form and maintain professional working relationships
* Calm, organised, flexible, approachable, enthusiastic, and punctual
* Good working knowledge and application of the MS Office packages
* Effective time management and organisation skills
* Able to manage difficult situations effectively.

* Ability to work autonomously and in a team
* Advanced IT skills
* Community experience
* Evidence of introducing change and innovation into nursing practice
