Qualified Social Worker - Hospital (Lancashire, Fylde & Wyre)
Rate: £33 per hour
Location: Fylde & Wyre, Lancashire

Eden Brown Synergy is seeking an experienced Qualified Social Worker to join hospital social work teams in Fylde & Wyre. This role involves supporting hospital discharge pathways, triaging referrals, and ensuring timely social care interventions for adults.
Responsibilities
* Triage referrals through the Single Point of Access (SPA) and support hospital discharge processes
* Apply eligibility criteria under the Care Act 2014
* Complete Continuing Health Care (CHC) assessments and reviews
* Conduct Mental Capacity Act (MCA) 2005 assessments
* Work closely with multidisciplinary teams to ensure smooth discharge and continuity of care
Essential Requirements
* Qualified Social Worker (SWE registered)
* Access to a vehicle and ability to drive (travel required)
* Previous experience in hospital discharge pathways and adult social care assessments
* Strong knowledge of Care Act eligibility criteria and MCA assessments
* Ability to work independently and manage a varied caseload
