We have an exciting opportunity for a compassionate and motivated Social Worker to join our specialist Cystic Fibrosis (CF) and Metabolic Bone teams. This role focuses on delivering high‑quality, family‑centred care to children, young people and their families within a multidisciplinary setting.
The CF team provides care for children and young people aged 0‑16 with cystic fibrosis, supporting families from diagnosis through transition to adult services. The Metabolic Bone team delivers specialist care to children with osteogenesis imperfecta.
Responsibilities
* Provide specialist social work support to children, young people and families affected by long‑term health conditions.
* Provide emotional, practical and professional guidance, promoting resilience and wellbeing.
* Undertake holistic assessments and develop family‑centred care plans.
* Work collaboratively within a multidisciplinary team to deliver coordinated, high‑quality care.
* Advocate for children and families to ensure their voices inform decision‑making.
* Support families through key stages including diagnosis, hospital admissions and transition to adult services.
* Maintain accurate records and contribute to service development and quality improvement.
Qualifications
* Qualified Social Worker with current Social Work England registration.
* Experience working with children and families, ideally within health or specialist services.
* Confidence in managing complex cases and working with long‑term or life‑limiting conditions.
* Strong communicator who values teamwork and partnership working.
* Compassionate, resilient and committed to delivering high standards of care.
