* £33,366 per annum (pro rata for part time)
* Post 1: 37 hours per week
* Post 2: 18.5 hours per week

Your role and responsibilities
As a Senior Independence and Wellbeing Practitioner, you'll be based at West Suffolk Hospital, working in a fast-paced acute hospital environment. Your focus will be on supporting timely and safe discharges, helping people regain their independence and wellbeing.
You will
* carry out Care Act Assessments and Mental Capacity Assessments
* assess functional ability and promote digital care technology
* provide advice and guidance to customers, families, and health professionals
* collaborate with NHS colleagues and social care teams to deliver joined-up support
* support community referrals to prevent hospital admissions
* manage referrals, including benefits tracking and eligibility decisions
You will also have the opportunity to work flexibly, including some home working. There is a requirement for weekend working, averaging 1 weekend in 5 which is shared across the team.
You will need

* A strong understanding of adult social care and the Care Act
* Experience working in a health or social care setting
* Excellent communication and decision-making skills
* A proactive, person-centred approach to supporting independence
