Position Overview

We have an opportunity for a forward‑thinking dynamic Practitioner within our LDA Enhanced Support & Liaison Service (ESLS). The role involves working across core hours, with occasional weekend work.

ESLS is a county‑wide, 7‑day a week service (8am‑8pm). It is part of a multi‑agency pathway that works with service users, providers, carers, families, social care and health partners to enable people with learning disabilities and/or autism to receive appropriate care in the right place at the right time.


Levels of Support

Level 1 – Generic support Level 2 – Patient specific support Level 3 – Enhanced patient specific


Pathways

* Community Liaison Pathway
* Inpatient/Discharge Pathway
* Enhanced and Intensive Community Intervention & Support


Role Focus

This advertised post focuses on providing direct clinical leadership and clinical expertise into the Enhanced and Intensive Pathway.


Responsibilities

* Deliver daily clinical leadership to a motivated team of health professionals in a fast‑paced pathway.
* Coordinate and plan person‑centred care for individuals and families in high levels of distress.
* Support others to provide core therapeutic interventions such as Positive Behavioural Support or mental health interventions.
* Lead clinical discussions and decision‑making in line with the Mental Health Act 1983, Mental Capacity Act and relevant legislation.


Qualifications & Experience

* Health professional with significant experience delivering clinical care and interventions to autistic people and/or people with a learning disability in acute mental distress.
* Experience of individuals at risk of admission to a mental health setting or placement breakdown.
* Sound knowledge of the National Dynamic Support Register, CETR policy and the national agenda to support this group of citizens.
* Ability to work within a multi‑agency pathway, collaborating with social care and health partners.
