Job Summary
We are looking to recruit a Band 6 Registered Nurse for the Hospital at Home service, providing urgent community response and virtual ward care to West Cheshire residents in their own home. You will be joining a multidisciplinary team including GP’s, Advanced Clinical Practitioners, nurses, therapists, pharmacists and clerical officers.
The role requires an enthusiastic, self‑motivated professional who can provide confident support, leadership and coaching to clients and clinicians, and who possesses excellent communication skills. You must be able to work in partnership with clients and families/carers, safeguard individuals at risk and support vulnerable patients.
It is a permanent post working 30 hours per week across a 7‑day service (8 am‑8 pm). Short‑listing is due 11 Dec 2025 with an interview date of 18 Dec 2025.
Main duties of the job
* Provide a highly visible clinical profile with leadership and clinical input for a designated clinical caseload.
* Collaborate with wider health, social care and third‑sector partner agencies to meet patient needs.
* Ensure patients receive the right care, by the right person at the right time and the right place.
* Place the patient at the centre of all decisions, reinforcing a streamlined journey with minimal avoidable delays and focusing on independence and quality of care.
* Provide direction within the caseload in consideration of local health and social care agendas and initiatives.
* Remain aware of local public health community profiles and how these influence required care.
* Focus on delivery of person‑centred care.
* Offer evidence‑based care to a defined population through caseload management and monitoring of care standards.
* Provide clinical leadership, coaching and development of the clinical team where appropriate.

Person Specification – Essential
Qualifications:
* Registered general adult nurse – first level
* Evidence of teaching and assessing
* SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling and application of health needs assessment
Knowledge and Expertise:
* Proficiency in delivering high‑quality evidence‑based care
* Demonstrate continuing professional development
* Ability to work and communicate effectively within an MDT setting
Experience:
* Significant experience of working within a community healthcare setting
* Experience of making autonomous decisions
