Overview

Partnership of East London Co-operatives (PELC) Ltd invites applications for the position of Clinical Streamer. The closing date is 24 October 2025.

There is a rising demand for Clinical Streamers within Urgent Care Services. PELC is a not-for-profit organisation delivering Urgent care at four Urgent Treatment Centres. Two main sites operate 24/7 at King George Hospital in Goodmayes and Queen’s Hospital in Romford. Both are part of the Barking, Havering and Redbridge University Hospitals NHS Trust, one of the larger acute trusts in the country. The aim is to improve waiting times for urgent and emergency care and performance against the four-hour emergency access standard.

We are GP-led, staffed by GPs, nurses and other clinicians, with access to simple diagnostics (e.g., urinalysis, ECG, X-ray, blood). Following initial assessment, we aim to deliver high-standard, patient-centred care with a focus on care principles and clinical safety. Clinical Streamers will receive training to support decision making in assessing the appropriateness of cases suitable for UTC and ED, with learning and support for protocols to manage emergency cases, critically ill and injured adults and children who arrive at an urgent treatment centre.


Main duties of the job

The UTC (Urgent Treatment Centre) and UCC (Urgent Care Centre) are staffed by a multidisciplinary team responsible for patients presenting who may require primary, urgent or emergency care. The team will see and treat patients according to agreed protocols and direct and refer patients to the most appropriate clinical area based on clinical needs. The Clinical Streamer role was developed in response to increasing self-presentation with health-related issues, directing patients promptly to the right place first time. The Clinical Streamer will ensure safe and effective redirection of patients to the most appropriate clinician or health care provider in accordance with approved guidelines. Hours of work: Clinical Streaming is fully operational 24 hours a day, 7 days a week including bank holidays.

Job responsibilities

Purpose of the role: The UTC/UCC is staffed by a multidisciplinary team who see and treat patients who may require primary, urgent or emergency care, according to agreed protocols, and direct and refer to the most appropriate clinical area. The Clinical Streamer directs patients promptly to the right place first time and ensures safe redirection to the appropriate clinician or health care provider in accordance with guidance.

Hours of work: 24/7 including bank holidays. Key relationships include UCC/UTC Service Managers, Primary Care Doctors, Nurses, Administrative staff, Local Primary Care teams, Community Nursing services, NHS 111 Mental Health professionals, Local Walk-in Centres, Social Services, GP practice staff, and London Ambulance Service.


Key responsibilities

Clinical

* Take clinical responsibility for yourself once fully oriented to the role and escalate governance issues.
* Direct patients to the most appropriate clinician or health care provider using clinical experience and expertise.
* Work as an autonomous practitioner and be accountable for all professional standards and guidelines.
* Provide direct access to patients seeking healthcare.
* Monitor the streaming queue to manage patients who may deteriorate or become acutely unwell.
* Conduct focused history taking with appropriate observations per local policies and audit criteria.
* Understand acutely unwell adults or children including red flags.
* Competent use of streaming equipment.
* Communicate effectively to ensure safe handover of patients to the Emergency Department.
* Redirect patients to named clinicians or other providers for best outcomes and counsel on health and social matters where needed.
* Assist patients and carers with language or cultural barriers to access services.
* Provide comprehensive handover at the end of each shift.
* Act as an advocate for the patient.
* Keep up to date with Safeguarding and Sepsis guidelines.

Developmental

* Participate in an individual performance review.
* Work with the multidisciplinary team to assess future nursing needs and developments within UCC/UTC.
* Assist in the development of the Clinical Streaming role.
* Participate in local research and clinical audits.
* Foster good relations and collaboration within health and social care sectors.

Professional

* Work within PELC policies and procedures and with due regard to the Code of Professional Conduct.
* Represent PELC, the UCC/UTC, and nursing to maintain high regard for these roles.
* Maintain documents and records in line with PELC and professional guidelines.
* Comply with NMC/HCPC requirements for professional registration and practice.
* Maintain required compliance updated annually.

General

Work in accordance with PELC and Trust policies and guidelines at all times. Adhere to Information Security Policy and ensure no confidentiality breaches. The job description is a guide and may be varied from time to time.


Person Specification


Qualifications

* Experience in Paediatric Emergency Department
* Commitment to developing and delivering innovative, high-quality primary healthcare services
* Nurse Practitioner or Emergency Nurse Practitioner Qualification or Paramedic or Physician Associate
* Experience in Community or Primary Care Setting
* Nurse/Prescriber/Non-Medical Prescriber


Experience

* A minimum of 2 years demonstrable experience of working in ED
* Understanding and knowledge of policy developments related to the delivery of Primary Care Services including General Practice
