Specialist grade General Medicine/ COTE / Movement Disorder

The post will be based in Ystrad Mynach, within the Medical Directorate to provide support for General Medicine, front door emergency care, Movement Disorder and general geriatric care to inpatients managing inpatient beds.

Specialist Grade doctors will work autonomously and independently to a high level of specialist knowledge and expertise within agreed lines of responsibility within the provisions of Aneurin Bevan's autonomous Practise policy.

Facilities: office accommodation, secretarial support and full IT facilities will be provided.

The number of SPA's may vary with the agreement of the post holder and the Health Board in advance or after commencement and an increase in SPA's would accompany an agreement to take on additional duties.

The successful candidate will have training access across the medical specialties and to a variety of accredited training courses, under the supervision of trainers. Audit and QI projects will be a requirement of the post and we have close, links with Cardiff University should you want to be involved in research.


Main duties of the job

To work collaboratively with Consultant colleagues in the development of services in Ysbyty Ystrad Fawr.

Specific responsibilities of the post are listed in the next section.

As a senior employee of the Health Board the post holder will work in close co-operation with, and support other clinical, medical professional and managerial colleagues in providing high quality healthcare to the Health Board's patients.


Job responsibilities

* Integral to the responsibilities of the post are the following requirements:
o To provide effective leadership
o To sustain and develop teaching and research wherever appropriate to service development
o To conduct clinical practice in accordance with contractual requirements and within the parameters of the Division's and Health Board's service plans
o Deliver and actively participate in the front door service (Monday to Friday) on pro‑rota basis and support general medical take during normal working day for YYF site.
o To provide delivery of Movement Disorder service for Caerphilly locality (inpatient and outpatient)
* Support and deliver the current establishment of a 7‑day service and further service delivery for the front door during daytime
* To participate in the senior decision‑making post – take ward rounds and provide senior advice to the staff in the Medical Assessment Unit during your sessions as pro‑rota. The post holder will be responsible for providing an on‑site presence acute care (2 sessions equivalent). This is based on the rota approved by the HR.
* To share the responsibility of inpatient general medicine with your consultant colleagues
* To run general/specialty outpatient clinics
* To share the responsibility for the care of elderly and acute geriatric patients on ward 3.2 (14 patients), as they require general medical care and MDT working
* Delivery of Movement Disorder service for Caerphilly locality
* To provide a presence on your clinical ward during your sessions leading ward rounds, board rounds and MDTs.
* To conduct clinical practice in accordance with contractual requirements within the parameters of the Divisions and Health Boards agreed objectives
* All clinical staff are accountable and responsible for their own clinical competence and should limit their actions to those for which they are deemed competent. Clinical staff are also required to comply with the requirements of their professional organisations regarding supervision
* Afternoon rest (30 minutes) has been incorporated in the job plan as per RCP guidance when doing 8 am to 4 pm sessions.
* Flexible training is welcomed and can be discussed with ADD or CD.
* Appointed speciality doctor will be given 1.6 SPA sessions (based on 10 sessions job), provided justified but this can be revised. There will be 0.5 admin session as part of the 8 core sessions. The job plan will be reviewed regularly to ensure that new consultants get appropriate time for professional development and CPD activities.
* On‑call speciality doctor is supported by an on‑call team including one StR / Senior clinical fellow (Registrar grade) and three FY2/GPVTs during day and two by night. There is one doctor (FY2/GPVTS) who cover twilight shift from 2 pm to 11 pm. There is one ANP during the day covering MAU (7 days a week) and additional resident to support MAU from 8.30 am to 4.30 pm, and two ANP Core Site Safety Team during nights.
* Morning post take ward rounds along with reviews of previous day patients in MAU starts at 8 am. The (Same Day Emergency Care) SDEC unit runs from 8 am to 7 pm Monday to Friday.
* Teaching/Training (as agreed with Clinical Director)
* To provide conditions for improved training opportunities in line with national and local recommendations arising out of MMC and related requirements
* To participate in the undergraduate teaching programme
* To assist and participate with the development of postgraduate training for F1s, F2s, SpRs, STs and other staff as appropriate
* Where agreed, to act as Educational Supervisor to junior and middle grade medical staff. New Appointee will be encouraged to take a formal Educational Supervisor role within HEIW and sign tripartite agreement formally after completing training.
