Essentials: - Significant post-qualifying experience in a similar role within adult care. - Extensive experience working within a fast-paced healthcare setting, ideally within an Integrated Discharge Team. - Proficiency in undertaking safeguarding assessments and working within the frameworks of the Mental Capacity Act. - Proven ability to deliver evidence in professional settings. - Registered with Social Work England with a valid social work qualification (e.g. DipSW, CQSW, CSS, Degree in Social Work).
