Charter Medical Centre and Trinity Medical Centre, part of Goldstone Primary Care Network, are looking for one or two enthusiastic individuals to join our team in a flexible part-time capacity. This role can be structured as a job share or split into two distinct part-time positions, depending on candidates' availability, strengths, and preferences.
We seek a proactive person committed to understanding and supporting our patients, capable of managing a caseload that may include dementia review support and outreach work for frequent attenders. The candidate should have a practical, empathetic, and independent approach, demonstrating dedication and a passion for patient support.
Social prescribing empowers individuals to take control of their health and wellbeing through referrals to non-medical link workers who focus on what matters to the individual and take a holistic approach. Link workers connect people to community groups and statutory services for practical and emotional support, helping to sustain existing groups and start new ones. This approach is particularly effective for those with long-term conditions, mental health support needs, loneliness, social isolation, or complex social needs.
Main duties of the job
1. Take referrals from a wide range of agencies, including GP practices within primary care networks and voluntary, community, and social enterprise (VCSE) organisations.
2. Provide personalized support to individuals, families, and carers to promote independence and improve health outcomes. Build trusting relationships by focusing on what matters to the individual, using a holistic approach based on their priorities and broader determinants of health.
3. Develop and co-produce personalized support plans, connecting individuals to community groups and statutory services as needed.
4. Manage and prioritize your own caseload, recognizing when to refer individuals back to other health professionals or agencies, especially when needs exceed the scope of the link worker role, such as mental health issues requiring qualified practitioners.
5. Enhance community capacity by supporting local VCSE organizations and community groups, ensuring they have safeguarding processes and can facilitate opportunities for social connection, skill development, and confidence building.

Person Specification
Qualifications
* NVQ Level 3, Advanced Level, or equivalent qualifications, or working towards this level.
* Commitment to ongoing professional and personal development.
* Training in motivational coaching and interviewing or equivalent experience.
* Willingness to complete NHS Learning for Link Workers training.
Experience
* Experience in community development, adult health and social care, learning support, or public health, including unpaid work.
* Experience supporting people, families, and carers in related roles.
* Experience working with the VCSE sector, volunteers, and community groups.
* Experience in partnership working and building organizational relationships.
* Experience in data collection and impact monitoring.
* Experience supporting mental health, whether paid, unpaid, or informal.

This role is subject to a DBS check under the Rehabilitation of Offenders Act (Exceptions Order) 1975, requiring a disclosure submission to verify any criminal convictions.
Salary Range: Between £29,970 and £36,483 per annum, pro-rata.
