Making a Difference as an Advanced Practitioner
We are seeking a qualified Assistant Team Manager / Advanced Practitioner / Social Worker to join our Integrated Health and Social Care team within Contact Cares, Hospital Discharge Service. This team is large, busy, and innovative, focusing on early intervention to improve lives, promote independence, and facilitate timely hospital discharges.
The team conducts assessments and care planning for adult patients, working closely with health and social care staff.
