Purpose of Role:
An exciting new opportunity has arisen for a Healthcare Plan Case Manager to join our team, supporting individuals through tailored, Trust Funded Healthcare Schemes. In this role, you will assess client needs, coordinate personalised interventions, and provide expert guidance to help clients achieve their health and wellbeing goals. Drawing on your professional background and strong communication skills, you will ensure each client receives the right support at the right time for sustainable recovery and long-term health.
Key Responsibilities:
* Establish the current reason for ill health injury and identify any underlying cause or contributory risk factors
* To be instrumental in the multidisciplinary assessment process by ensuring information ins gained sufficient to assess personal and occupational risk factors
* To effectively communicate to the member, the multifactorial issues associated with the current episode of ill health/injury and how they can personally address the risk factors within their control
* Where appropriate review both private and NHS options when making onward referrals for consultations, investigations and treatment
* Source appropriate treatment providers whilst always adhering to guidelines outlined in the Rules & Benefits
* Always manage claims within the restraints of the Rules & Benefits
Person Specification:
Experience
Experience conducting telephone assessments and feedback using motivational interviewing or similar techniques
Skills & Knowledge
Proficient in MS office packages
Strong communication skills both via telephone and email
CBT principles and motivational interviewing techniques
Problem solving skills
Attributes
Good attention to detail
Strong clinical knowledge
Qualifications and Training
English and Maths GCSE Grade C or above
