Lakenham Surgery is seeking a Clinical Pharmacist to join the practice on a part-time basis. This is an exciting opportunity to join an incredibly friendly and supportive multi-disciplinary practice team delivering high quality patient care to our patient population in the centre of Norwich.
Working collaboratively alongside a range of healthcare professionals and support staff within the practice and Primary Care Network (PCN), the Clinical Pharmacist will help to support delivery of the Primary Care Network (PCN) Directly Enhanced Service (DES), local ICB and practice based Prescribing Initiatives such as the Prescribing Quality Scheme, and generally focusing on the optimisation of medicines management and prescribing for our patient population.
Main duties of the job
The successful candidate will
* Work collaboratively as part of a multi-disciplinary team within the practice and wider Primary Care Network helping to support delivery of the Primary Care Network (PCN) Directly Enhanced Service (DES) and high quality patient services
* Support the implementation of Prescribing Quality Scheme initiatives and the improvement of prescribing practices within the practice and across the PCN
* Conduct structured medication reviews to facilitate the proactive management of patients with complex polypharmacy
* Provide clinical medicines expertise to both patients and practice teams
* Improve patient service and quality through mechanisms such as review and clinical audits
* Work collaboratively with the Prescribing Support Team (Prescribing Care Coordinators) to implement improved processes for the management of prescriptions within the practice and the optimisation of patient care
* Liaise as required with community and hospital colleagues to help resolve medication-related issues
* Play a leading role in supporting the practice to review and manage patients on dependency forming medication

Person Specification
Experience
* Able to apply clinical knowledge appropriately in a variety of practice settings or patient groups
* Experience of clinical pharmacy
* Good critical appraisal skills
* Experience of patient consultations
* Good understanding of primary care structure and General Practice
* Good clinical knowledge
* Post-registration experience in primary or secondary care
* Knowledge of GP clinical computer systems
Qualifications
* Registered with the General Pharmaceutical Council
* Non-medical prescriber registration or willingness to undertake Prescriber training
* Evidence of Continuing Professional Development
