Job overview Surrey and Borders Partnership NHS Foundation Trust is looking for someone who is passionate about ensuring a high quality holistic approach to the care and support provided to people with learning disabilities in the community who have been identified as in need of community nurse support to enable them to exercise choice over their own lives. Location: 18 Mole Business Park, Randalls Road, Leatherhead, KT22 7AD Hiring 2 registered Nurses. Work patterns differ: 1) Monday to Friday 9AM to 5PM 2) 3 days a week - Flexible Advert Provide clinical leadership and supervision to Band 5 nurses and support workers, contributing to appraisal and performance development. Deliver specialist learning disability nursing care, including assessment, care planning, risk management, and crisis intervention. Act as an advocate for people with learning disabilities, ensuring person-centred, dignified, and inclusive care. Coordinate care for individuals under the Care Programme Approach (CPA) in line with Trust policy. Work collaboratively with multidisciplinary teams, primary care, acute services, and external agencies. Ensure practice complies with professional, legal, and safeguarding frameworks, including the NMC Code, Mental Health Act, and Mental Capacity Act. Contribute to service development, quality improvement, policy implementation, and national initiatives. Support recruitment, induction, education, and training of staff and students. Participate in audit, research, and quality assurance to improve service delivery. Travel across community settings as required to deliver care. Detailed job description and main responsibilities Leadership and Service Development Provide day-to-day clinical supervision and contribute to Individual Performance Reviews/Appraisals for Band 5 nurses and support workers. Communicate the healthcare needs of people with learning disabilities to the Team Manager, Professional Lead, and Care Practitioners. Advocate for the service internally and externally and contribute positively to divisional and strategic nursing developments. Support staff recruitment, induction, and workforce planning, particularly in relation to nurse recruitment. Contribute to the development, implementation, and review of policies, protocols, and care pathways. Promote equality, diversity, and inclusion by ensuring the needs of people with protected characteristics are reflected in service delivery and future developments. Clinical Responsibilities Demonstrate advanced specialist nursing knowledge in learning disability practice, including: Crisis intervention Alternative and augmentative communication Mental health assessment and management Epilepsy care Medication management Management of challenging behaviour Undertake comprehensive assessments and develop, implement, and evaluate person-centred nursing care plans across a range of community settings. Actively involve individuals and their families in care planning, ensuring dignity, choice, cultural sensitivity, and respect for personal preferences. Practice within the Positive Behaviour Support (PBS) framework. Complete detailed risk assessments and develop robust risk management plans. Undertake the Care Co-ordinator role for individuals under the Care Programme Approach (CPA), where appropriate. Build and maintain effective working relationships with MDT colleagues, partner agencies, service users, and families. Professional, Legal, and Educational Responsibilities Maintain professional and legal accountability for all aspects of clinical practice in line with the NMC Code, Trust policies, and professional guidelines. Work within the Mental Health Act, Mental Capacity Act (2005), and related legislation. Implement Safeguarding Adults and Child Protection policies and fulfil statutory safeguarding responsibilities. Act as a practice assessor/teacher, supporting pre-registration students, junior staff, and new starters, and contribute to the audit of learning environments. Research national best practice and support benchmarking against recognised standards in partnership with people with learning disabilities. Participate in clinical audit, research, and quality assurance activities, maintaining up-to-date professional knowledge. Develop, deliver, and evaluate specialist training packages for multidisciplinary audiences. Support the implementation of national and government-led initiatives, including Transforming Care and Learning Disability Mortality Reviews, to improve health outcomes. Additional Requirements Due to the nature of community-based services, the post holder must hold a full UK driving licence and have access to a vehicle for work purposes.
