We are seeking a motivated, innovative, and experienced Physiotherapist, registered with HCPC, with proven clinical and leadership skills, excellent communication skills, in an autonomous role, to join our professional and friendly multidisciplinary Advanced Practitioner team.
This is a patient-facing role involving evidence-based practice, clinical reasoning, and skills to assess, plan, treat, evaluate, and refer patients appropriately.

Main duties of the job
Key responsibilities include:
1. Assessing patients via self-referral or internal clinical referrals, developing care plans, and providing initial treatment options.
2. Supporting MSK clinical and service development across the PCN, collaborating with secondary and community care & MSK services.
3. Developing skills in independent prescribing, injection therapy, and investigations to make professional judgments and decisions, reducing reliance on pharmacological interventions.
You will be HCPC registered, hold a degree in Physiotherapy, and have practical experience at Level 6 capability in MSK, with excellent communication skills and adaptability to primary care demands.

Post Overview:
The post holder will provide musculoskeletal physiotherapy, including neck and low back pain management, using the AQP process. They will deliver high-standard care for patients with complex conditions, performing specialized assessments, diagnosis, and advice for an independent caseload.
The role involves collaboration with the physiotherapy team, supporting policy and service development, and the ongoing delivery of physiotherapy services. An enhanced DBS check is required.
Key responsibilities:
* Assess patients, design, and implement treatment according to clinical guidelines and CSP standards.
* Ensure patient understanding, obtain informed consent, and work within a legal framework.
* Maintain accurate patient records in line with CSP standards.
* Conduct comprehensive assessments of patients with complex conditions, using clinical reasoning and manual techniques to diagnose accurately, including interpretation of investigations.
* Develop and deliver specialized treatment programs based on evidence-based practices, including manual therapy, patient education, and exercise.
* Formulate prognoses, recommend interventions, and develop discharge plans, liaising with GPs and specialists as needed.
* Monitor progress, reassess, and modify treatment plans accordingly.
