The Neighbourhood Team consists ofOccupational Therapists, Community Nurses, Physiotherapists, Senior NursePractitioner, Healthcare Assistants and Administrators. GP and social carecolleagues work closely alongside.
TheNeighbourhood Team operates from 7:30am to 18:30pm on a seven-day rotaincluding weekends and Bank Holidays. You will be supported to work flexiblywithin these hours.

This is full time post, but we wouldconsider applicants looking for part time.(minimum 30 hours per week)
Main duties of the job
Yourtypical working day will include triage and allocation of planned and unplannedvisit requests for occupational therapy interventions. This may involve urgentresponse assessment as part of the teams admission avoidance remit, supportingcomplex hospital discharges and planning domiciliary interventions tofacilitate independent living. You will work alongside occupational therapy, nursingand physiotherapy colleagues in the same team. The role includes mentoring occupationaltherapy students. We have close links with Worcester and Coventry universitiesin support of these student placements.

Job responsibilities
The post holder will work as an autonomouspractitioner within an interdisciplinary Neighbourhood team providing rapid andexpert professional assessment and treatment interventions to patients eitherin their own homes or other community settings to avoid inappropriate hospitaladmission or to facilitate early discharge. This will be achieved through acombination of hands on expert clinical skills as an Occupational Therapist withknowledge to treat, educate and support patients; and by working with patientsand their families to assess, plan and implement appropriate packages of careand treatment
Providecase management for a designated caseload of patients who have immediate and /or complex care needs.
Triage patients to ensure prioritization ofclinical need.
Work holistically with all team members,carrying out joint visits and actively participating in the integration ofservices increasing the knowledge of all team members through case discussionand joint working

Person Specification
Qualifications
* Diploma/ degree in Occupational Therapy
* Health Professionals Council registration
* Relevant validated clinical courses
Knowledge
* A range of evidence based Occupational Therapy clinical skills and knowledge appropriate to specialism
* Understanding of professional and ethical boundaries and code of conduct
* Knowledge of the roles and responsibilities of the professional groups within an integrated team
* An understanding of the approaches and models of practice relevant to practice.
* A good level of knowledge in a wide range of physical and mental health conditions
* Knowledge of rehabilitation concepts and services
* Good level of knowledge relating to disability, equipment and adaptations
* A working knowledge of Occupational Therapy Outcome Measures and Models of Practice
* Working knowledge and experience of Occupational Therapy standardised assessments appropriate to the role
Experience
* Application of the Occupational Therapy process
* Use of functional assessment and activity analysis
* Experience of working in an integrated team
* Experience implementing change in clinical practice
* Experience in rehabilitation settings
Skills & Abilities
* Ability to work proactively with other agencies to prevent hospital admission and facilitate early discharge
* Ability to identify and plan individual care / rehabilitation needs in partnership with patients/ their carers
* Ability to make decisions autonomously, but liaise closely with and work across the multidisciplinary integrated team
* Ability to organise, prioritise and time manage own workload including use of effective delegation
* High level of clinical knowledge and skill
* Ability to communicate clearly and effectively in English in both verbal and written formats
* Awareness of scope of practice

* Able to meet the travel requirements of the role.
