An exciting opportunity has arisen for a Community Advanced Practitioner (CAP) to join the integrated team at Aberdeen City Health and Social Care Partnership. This is a permanent, full-time Band 7 position, ideal for a dynamic and experienced clinician looking to make a meaningful impact in community care.
About the Role
As a Community Advanced Practitioner, you will:
1. Provide direct clinical management of patients within the community nursing caseload.
2. Support community teams with complex palliative care
3. Conduct comprehensive assessments, formulate differential diagnoses, and deliver evidence-based treatment and management plans.
4. Collaborate closely with multidisciplinary teams to ensure high-quality, person-centred care.
5. Support the teaching, supervision, and development of nursing colleagues in advanced practice.
About You
We are looking for an energetic, compassionate, and motivated individual who thrives in a collaborative and evolving environment. You should have:
6. A PgDip or MSc in Advanced Clinical Practice.
7. Essential community experience and a strong foundation in advanced clinical skills across a broad range of settings.
8. A commitment to continuous improvement and service development.
