Location: Delivering services primarily within the Horden community, with a base at Peterlee Health Centre.
Working Hours: 37 hours per week
Contract Type: Permanent
Salary: £25,110 - £32,090

About the role

As a Dual Diagnosis High Intensity Worker for County Durham Drug and Alcohol Services you will be supporting people living with co-occurring mental health disorder and a substance use disorder.

This support will include advocacy, assertive outreach within the community and supporting clients to appointments where necessary.

You will carry a small caseload and be part of a team which works in a holistic way to support individuals experiencing multiple disadvantage – specifically co-occurring mental health and substance misuse issues - to improve their circumstances.

Key accountabilities

- Build a rapport with dual diagnosis clients experiencing multiple disadvantage that enables you to develop the relationship required to be able to deliver a person-centred, holistic approach.

- Make decisions in the work, with the individual, to reduce risk and meet needs.

- Manage financial resources to enable you to enact the decision you make, with the individual, where current provision does not exist/is not available

- Convene multi-disciplinary team (MDT) meetings to ensure appropriate coordination of partner agency support to avoid service duplication or conflicting support offers to influence positive outcomes for individuals

- Build relationships with other support providers, services, partners and agencies, including statutory services.

Essential criteria

- NVQ Level 3 in Health & Social Care (or above or equivalent experience) or willing to work towards this in post.

- GCSE passes including Maths and English at Grades AC or Level 9-4, or equivalent level of experience within a similar environment

- Understanding of issues relating to multiple disadvantage

- Previously worked with individuals experiencing multiple disadvantages

- Demonstrable experience of putting the person first and seeking out the best way to support families or individuals to improve their current situation.

Please note due to the nature of this role, a full UK driving license and access to a vehicle is essential .
